WURS-25

Wender Utah Rating Scale

A retrospective self-report measure of childhood ADHD symptoms. Think back to when you were a child (ages 6-12) and rate how well each statement described you.

Instructions

Answer based on how you actually were as a child, not how you are now.

Note: This is a retrospective screening tool, not a diagnostic instrument. It assesses recalled childhood symptoms as part of a broader ADHD assessment process.

What This Means

Important Note

The WURS-25 measures recalled childhood symptoms and is not a diagnostic tool for current ADHD. Retrospective recall can be affected by current mood, memory, and other factors. A comprehensive ADHD assessment includes current symptom measures, developmental history from multiple sources, and evaluation of functional impairment. Only a qualified professional can diagnose ADHD.

About the WURS-25

The Wender Utah Rating Scale (WURS-25) was developed by Paul Wender and colleagues at the University of Utah. It is a retrospective measure that asks adults to recall their childhood experiences and behaviours. Because ADHD is a neurodevelopmental condition that must be present from childhood, establishing a developmental history is an essential part of adult ADHD assessment. The WURS-25 provides a standardised way to assess this history, with a clinical cutoff of 46 that has shown good sensitivity and specificity for identifying adults who had childhood ADHD.
